The subscription service is offered on a monthly basis only. All financial transactions will occur though a 3rd party service. Patient videos can only be sent under the direction of a licensed health care provider (i.e., a dentist or physician).
The monthly payment for the subscription service must be received prior to the start of the month (i.e., in advance). If, for any reason, a monthly payment is not received prior to the start of the month, we may either suspend or terminate your subscription service.
The monthly fee for the subscription service may change from time to time. We will notify you in writing at least 30 days in advance when the monthly fee changes.
You may terminate your subscription service at any time by notifying us in writing. No refund for the subscription service will be returned to you for the month in which the termination occurred. Although, your access to the subscription service may immediately cease as soon as we receive your termination notice.
We may terminate your subscription service at any time for any reason or for no reason. This termination may occur without notice and/or without warning, although we may attempt to notify you of the termination in writing. No refund for the subscription service will be returned to you for the month in which the termination occurred, and your access to the subscription service will immediately cease upon this termination.
Upon termination of your subscription service for any reason, we may immediately delete your account, including any data or information that you provided regarding the subscription service.
Unless otherwise indicated, the website and videos, hereinafter “Content,” are the copyrighted property of Patients-Path, LLC. No part of the Content may be copied, reproduced, republished, uploaded, posted, publicly displayed, transmitted, or otherwise used for any commercial purpose whatsoever without our express prior written consent.
The videos emailed to patients do not provide medical advice. They only provide general information to help the patient understand a particular medical condition. You consent not to represent these videos as medical advice.
When a video is emailed to a patient, you consent to ensure that the indicated patient is the same person that is recorded in the patient’s record.
These Terms of Use and your use of the website are governed by the State of Michigan. You expressly agree that exclusive jurisdiction for any dispute with Patients-Path, LLC, or in any way relating to your use of the Patients-Path website, resides in the state or federal courts of Oakland County, Michigan.
You agree to defend, indemnify, and hold Patients-Path, LLC, its officers, directors, employees, agents, licensors, and suppliers, harmless from and against any claims, actions or demands, liabilities and settlements including without limitation, reasonable legal and accounting fees, resulting from, or alleged to result from: (a) your use of the website, (b) your use of or reliance on any Content, or (c) your violation of these Terms of Use.

Without limiting the foregoing, Patients-Path, LLC, its licensors, and its suppliers make no representations or warranties about the accuracy, reliability, completeness, or timeliness of the Content.
In no event shall Patients-Path, LLC, its licensors, or its suppliers be liable for any damages (including, without limitation, incidental and consequential damages, personal injury/wrongful death, lost profits, or damages resulting from lost data or business interruption) resulting from the use of or inability to use the Content, whether based on warranty, contract, tort, or any other legal theory, and whether or not Patient-Path, LLC, its licensors, its suppliers are advised of the possibility of such damages.
