We collect the name and payment information of the subscriber (doctor or other individual on doctor’s behalf) for each account. The payment information may consist of either credit card or PayPal account details. The administrator may add one or more team members to each account, which consists of the name of each team member. The administrator may also add one or more patients to each account, which consists of the patient’s name, birth date, telephone number (or other 10 digit code as designated by the patient) and email address.
We do not sell or share your information with anyone for marketing purposes. We may share your information for the purposes of improving our website or to comply with the law.
